Lost or Damaged Registration or Plates
A duplicate registration can be obtained at any DMV branch location or at a AAA branch office (members only).
Forms and Fees
- You need to complete and sign the Application for Registration.
- Refer to the third page of the Application for Registration for a list of documents needed to perform this transaction.
- Costs are listed in the DMV Fee Table
Lost or Damaged Registration
- By Mail: Send the Application for Registration to the Division of Motor Vehicles Attn: Registration Duplicate, 600 New London Avenue, Cranston RI 02920
Lost/Stolen Plates
- You must surrender plate(s) to get new ones.
- Vehicles issued two plates: If both plates are lost/stolen, you need to provide a police report. You can file the report at your local law enforcement agency. Or complete the Affidavit for Cancellation of Registration for Lost Plates.
- Vehicles issued one plate: You must provide a police report for the lost/stolen plate. You can file the report at your local law enforcement agency. Or complete the Affidavit for Cancellation of Registration for Lost Plates.
Damaged Plates
- Damaged Plates: Mail the Application for Registration and the Application for Remake of License Plates form with the appropriate payment to the Division of Motor Vehicles Attn: Plates, 600 New London Avenue, Cranston RI 02920.
Replacement Plate Stickers: you must bring your current registration certificate to any DMV branch location or AAA branch office (members only) to obtain a new set of stickers. There is no charge for replacement stickers.
