Name Change

Name Change for your Registration
  • To change your name on your registration, you must go to one of the DMV locations with your new license, (the temporary license with your new name will suffice) your title, your legal document, and proof of insurance. You will be asked to fill out the TR-1 form pdf and the cost will be $31.50. This cost covers the name change on both your registration ($5.00) and your title ($25.00) plus 1.50 technology surcharge fee.
  • To change your name on your title: if the title is in your possession, sign the title over to yourself by signing and printing your former name on the back of the title as "seller". Then sign and print your new name as "buyer". You must have an original copy of the title.

 

Name Change for your License

To change your name on your RI license, you need to appear in person at any DMV branch office during regular business hours, and bring the following:

  • Your current RI license
  • One document showing your correct name, such as a marriage certificate or divorce decree.

OR

  • The DMV will accept a notarized affidavit stating the change of name. This affidavit is available in any DMV branch and must be signed by you and notarized.
  • Along with this affidavit, you must provide the DMV with two documents evidencing the new name, such as a Social Security card, a valid voter registration card, or current U.S. passport.

You are required to change your name with Social Security first. Please allow at least 24 hours for Social Security to make the change to their records before changing your name with the DMV. If the name change is not in the Social Security system, the DMV cannot process your name change on your license.

The fee to update your license is $6.50.
