Lost Registration

If you have lost your registration certificate and need a duplicate, you must first complete the application (TR-1) form. The form must be signed by you and notarized, and your insurance information must be complete. The fee for a duplicate registration certificate is $18.50.

Mail the completed application with a check for $18.50 to RI Division of Motor Vehicles, 100 Main Street, Pawtucket, RI 02860

 

Lost or Damaged Plates

If you have lost one or both plates, or have damaged plates, you must complete a plate change transaction.

If your current plates are damaged and you want to order a new set with the same registration numbers, please fill out a

Wave Plate Order form or Designer Plate Order form and mail it to RI Division of Motor Vehicles, 100 Main Street, Pawtucket, RI - Attention PLATES. When your plates are ready to be picked up you will be notified by mail. You must return the two damaged plates you currently have. If you have lost one or both plates, you must have a copy of a police report in order to cancel those plates.

You must send payment with your remake order - check or money order only - PLEASE DO NOT SEND CASH.

If you have lost one or both plates and wish to immediately obtain a set of plates, any registration number, you must provide the DMV with a police report number in order to cancel those plates, then fill out the form below. The fee for this plate change is $21.50.

• Fill out a TR-1 application
• For a $21.50 fee (more if a vanity plate) you will receive your new plates, registration and expiration stickers.
